Increase monitoring frequency. Notify senior staff. Document rationale.

High Risk

Contact GP or clinical lead within the hour. Begin escalation protocol.

Emergency

Call 999 immediately. Do not delay. Use the ASK Technique™ script.

The Escalation Decision Ladder™ sets the required action. Staff use the ASK Technique™ script to make the call clearly and confidently. Everything is logged with time, user and outcome.
